Laser Hemorrhoidoplasty Procedure for Second, Third, and Fourth Degree Hemorrhoids: A Single Center Prospective Study

Abstract
OBJECTIVE: To study the results of hemorrhoidal laser procedure (HeLP) in patients diagnosed with grade 2, grade 3, and grade 4 hemorrhoids. STUDY DESIGN: This study includes the results of patients diagnosed with symptomatic grade 2, 3, and 4 hemorrhoids in a single center between January 2018 and January 2019 and treated with HeLP. Written informed consent was obtained from all patients. The patients were divided into 3 groups according to severity of their condition: grade 2, 3, or 4 according to the Goligher classification. There were 40 patients in each group. All data belonging to patients were randomized and recorded prospectively. RESULTS: Of the 120 patients, 80 were male (66.6%) and 40 were female (33.4%). Operation times averaged under 20 minutes in all groups. Thrombus was seen in 8 (20%) patients; 2 of them were drained, while the others healed with medical treatment. In the grade 4 hemorrhoids group, grade 2 level prolapse was observed in 10 (25%) patients despite its regression compared to the preoperative period. Postoperative complications in the grade 4 hemorrhoids group were found to be statistically significant as compared to the other groups (p<0.001). Four (10%) patients recurred in the grade 4 hemorrhoids group and were treated with traditional surgery (p<0.001). CONCLUSION: The laser hemorrhoidoplasty method is an easily applied, safe, effective, and high-patient-satisfaction procedure, although multicenter and large series studies are needed. © 2020 Science Printers and Publishers Inc..
